These before and after photos show the natural looking results of lower eyelid rejuvenation surgery one year after treatment. This lady had fat grafting to the cheeks and a pinch of skin removed along the lower eyelids.

Lower eyelid surgery is a special interest of mine. I have spent time studying with several leaders in this field. This includes Dr. Feigan (Florida), Dr. Baker (NYC), Dr. Zeltser (Belgium), and Dr. Mendelson (Australia). I have publish research on lower eyelid surgery in the Journal of Plastic and Reconstructive Surgery. Treatment plans are individualized for each person. As we age the cheeks drop. This is due to gravity, shrinkage of cheek fat, and loss of cheek bone. As this occurs the lower eyelid becomes longer. The distance between your eyelashes and the start of the cheeks increases. This also leads to dark areas under the eyes.

There are a number of options to increase lift the cheek and increase cheek volume. These will decrease the length of the lower eyelids and produce a more youthful and rested appearance. Volume can be added by shifting the upper eyelid fat into the area of the eyelid cheek junction (fat transposition). Fat from the abdomen can be transferred into the cheek (fat grafting). Fillers (such as Juvaderm) can be placed into the cheek. The most dramatic method is to use a cheek / lower eyelid implant. In some patients a small amount of upper eyelid skin is also removed.

After surgery there is bruising for 1 – 3 weeks depending on the treatment used.

Upper eyelid surgery (#blepharoplasty) was done for this 76 year old woman to improve her vision. In order to determine the best treatment a board certified plastic surgeon must asses the:

  1. brow (area above the eyes)
  2. the upper lid sulcus (the area below the eyebrow and above the eyelid)
  3. the upper eyelid skin
  4. upper eyelid fat
  5. position of the lacrimal (tear gland)
  6. position of the upper eyelid skin crease (tarsal fold)
  7. position of the eyelid margin (place where the eyelashes sit)

Age produces a number of possible changes. These include:

  1. brow descent
  2. hollowing of the sulcus
  3. nasal fat pad protrusion
  4. lacrimal gland descent
  5. descent of upper eyelid skin excess
  6. loss of the distinct tarsal fold
  7. descent of the eyelid margin (#ptosis)

Possible interventions include:

  1. brow lift
  2. adding volume to the sulcus
  3. elevation of the lacrimal gland
  4. removal or transposition of the nasal fat pad
  5. removal of upper eyelid skin excess
  6. recreation of the tarsal fold, and ptosis repair

Working with @drfaigen, @drmendelsonmelbourne, and dr Zeltzer @life_clinic_brussels I have studied the anatomy, assessment, and treatment of eyelid disorders thoroughly during my ten years of plastic surgery training after medical school.
